Kittitas County Sheriff's Office has developed 42 CFR part 2 compliant consent forms that <br />encompass all requirements set forth by the Legal Information Institute and the Legal Action <br />Center.All staff are fully trained in the utilization of consent forms prior to start of work. <br />Ongoing training is provided annually,or sooner,as rules change.The elements of each consent <br />are fully understood.We utilize two types of consent forms:Consent to Treat and Consent to <br />Release Information.Each consent form has the Prohibition on Redisclosure statement.All <br />participants will be fully informed of their right to sign,revoke,and/or limit the consent form. <br />These forms will be maintained within participant file and date/event of expiration will be <br />monitored.The initiating site will maintain the original consent form and a copy will be provided <br />to the referent.No participant information will be disclosed without a valid consent form being <br />completed,unless otherwise allowed by law (medical emergency,mandatory duty to report, <br />etc.). <br />Under 42 CFR Part 2,Subparts 34 and 35 disclosures may also be made to other correctional <br />facilities and to judicial agencies when participation in a program is a condition of their release <br />or parole.All elements of these subparts will be followed and documented within the participant <br />file. <br />Capabilities and Competencies <br />Our initial concerns focus primarily on being able to appropriately staff the jail with qualified <br />and culturally informed personnet A MAT partner organization will need to have developed a <br />list of appropriate candidates for the positions needed with the ability to have a full staff no later <br />than 12/01/2020 and will be able to provide services by 12/31/2020. <br />Kittitas County Sheriff's Office will seek a MAT partner organization that consists of <br />medical providers carrying certifications in Addiction and Emergency Medicine,Internal, <br />12 of 20
