The result of this investigation sha[[ be reported in this recommendation <br />of award [etter and printed copies of the search results are inctuded herewith. <br />Along with the signed Bid Proposal form, Belsaas & Smith Construction, lnc. submitted the <br />fottowing: <br />. Bid Bond in the amount of 5% of the Bid with Power of Attorney. The surety for the Bid Bond is <br />Travelers Casuatty and Surety Company of America, who is [isted on FederaI Circular 570 (NAIC <br />#19038) and licensed to do business in the State of Washington to an underwriter's limit of <br />$670,641,OO0. We have verified this information with the U. S. Department of the Treasury's <br />Listing of Sureties Approved to Conduct Business in the State of Washington <br />(https://www.fiscat.treasury.gov/surety-bonds/[ist-certified-companies.html). <br />' Compteted Local Agency Subcontractor List. <br />. Compteted and signed Locat Agency Proposal Bond. <br />. Completed and signed Wage Law Comptiance. <br />. Non-CottusionDectaration. <br />. Completed and signed Local Agency Proposal Signature Page. <br />. Acknowledgement of receipt for Addendum No. 1, Addendum No. 2, Addendum No. 3, and <br />Addendum No.4. <br />Based on the information before us, Belsaas & Smith Construction, lnc. has submitted the lowest <br />responsive, responsible bid. We therefore recommend that, if a Contract for this work is awarded, it <br />be awarded to Betsaas & Smith Construction, lnc. for the Base Bid total of $2,467,381.7O. <br />Should you have any questions or require additional information, please feet free to contact Tom <br />Parker at the number provided betow.
