Ellensburg Community Health Clinic Kittitas County Public Health Department HAHC Strengthen linkages between agencies providing services to homeless individuals/families Coordinate with agencies and study the use of a universal database management system in Kittitas County to provide better tracking and more efficient use of resources for the homeless populations. December 2009 HAHC with appropriate agencies Additional Case Management to Homeless Populations Encourage and support agencies in their expansion of case management services to homeless persons in transitional and/or permanent housing.
