DETIVERABTES <br />Required Grant Activity Strategies Measures <br />ldentify and screen individuals <br />who are at risk of SUD/OUD <br />and make available prevention, <br />harm reduction, early <br />intervention services, referral <br />to treatment and other <br />supportive services to <br />minimize the potential for the <br />devel opment of SUD/OUD <br />Create pathways <br />(screening, referrals, care <br />coordination) to treatment <br />services for people <br />accessing syringe exchange <br />programs <br />a Number of people referred <br />to treatment <br />a <br />Track, screen, prevent, and <br />refer to treatment patients <br />with SUD/OUD who have <br />infectious com pl ications, <br />including HlV, viral hepatitis, <br />and endocarditis, particularly <br />among PWID. <br />Conduct harm reduction <br />education and <br />HIV/hepatitis C testing in <br />settings with high risk <br />individuals (at least 4 <br />events per year) <br />a Number of education <br />participants <br />Number of patients tested <br />a <br />a <br />lmplement year-round drug <br />take-back programs. <br />Conduct pharmacist <br />education about drug take <br />back (at least 3 <br />pharmacists per year) <br />a Number of pharmacies <br />contdcted about drug take <br />back <br />Number of pharmacies <br />conducting drug take back <br />a <br />a <br />Enable individuals, families, <br />and caregivers to find, access, <br />and navigate evidence-based <br />and/or best practices for <br />affordable treatment and <br />recovery support services for <br />SUD/OUD, including home <br />and community-based <br />services and social supports <br />such as transportation, <br />housing, child care, legal aid, <br />employment assistance and <br />case management. <br />Conduct education with <br />large employers about <br />employing people in <br />recovery <br />Establish formal <br />partnerships with local <br />employment assistance <br />organizations <br />(WorkSource, People for <br />People, HopeSource)to <br />increase opportunities for <br />job assistance to people in <br />a <br />a <br />a <br />a <br />Number of large <br />employers receiving <br />education <br />Number of new <br />partnerships with <br />employment agencies <br />Except as set forth in this Amendment, the Contract is unaffected and shall continue in full force <br />and effect in accordance with its terms. lf there is conflict between this amendment and the <br />contract or any earlier amendment, the terms of this amendment will prevail.
