GRANT ADMI NISTRATION CONTRACT <br />AMENDMENT #1 <br />THIS GRANT ADMINISTRATION CONTRACT Amendment ("Amendment") is made by Kittitas <br />County Health Network ("Manager") and Kittitas County Public Health Department <br />("Contractor"), parties to the Grant Administration Contract ("Contract") for Services for Kittitas <br />County Opioid Use Disorder Prevention, Treatment, and Recovery Project effective September 1-, <br />2019 ("Effective Date") and, unless earlier terminated, terminate on August 3L, 2022 <br />("Termination Date"). <br />AMENDMENTS <br />The Contract is amended as follows: <br />Under "Recitals" section C, #L Project Scope of Work is changed to read (see bold text for <br />addition): <br />t. Proiect Scope of Work. Contractor will be responsible for assisting with the administration <br />of the Grant activities and services in Kittitas County, over which Manager has jurisdiction <br />as set forth in the Grant ("Services").
