GRANT ADMI NISTRATION CONTRACT <br />AMENDMENT #1 <br />THIS GRANT ADMINISTRATION CONTRACT Amendment ("Amendment") is made by Kittitas <br />County Health Network ("Manager") and Kittitas County Public Health Department <br />("Contractor"), parties to the Grant Administration Contract ("Contract") for Services for Kittitas <br />County Opioid Use Disorder Prevention, Treatment, and Recovery Project effective September 1-, <br />2019 ("Effective Date") and, unless earlier terminated, terminate on August 3L, 2022 <br />("Termination Date"). <br />AMENDMENTS <br />The Contract is amended as follows: <br />Under "Recitals" section C, #L Project Scope of Work is changed to read (see bold text for <br />addition): <br />t. Proiect Scope of Work. Contractor will be responsible for assisting with the administration <br />of the Grant activities and services in Kittitas County, over which Manager has jurisdiction <br />as set forth in the Grant ("Services"). Contractor agrees to increase community access to <br />Naloxone, conduct education about co-prescribing naloxone with opioid prescriptions, <br />connect syringe service clients with treatment services, conduct harm reduction <br />education, conduct education about drug take back services, and work to increase access <br />to employment opportunities for people in recovery. <br />Exhibit 2: Services and Deliverables table is updated as follows (see bold text for additions) <br />Required Grant Activity Strategies Measures <br />Develop, implement, and <br />assess intervention models <br />that leverage opioid overdose <br />reversal and increased <br />naloxone availability as a <br />bridge to treatment and <br />ensure that rural communities <br />have sufficient access to <br />naloxone. <br />Provide annual <br />educational information to <br />all Kittitas County <br />physicians to encourage co- <br />prescribing of naloxone <br />with opioid prescriptions <br />lncrease pharmacist <br />distribution of naloxone <br />Work with local pharmacies <br />to provide over the counter <br />naloxone with opioid <br />prescriptions <br />Distribute naloxone at local <br />a <br />a <br />a <br />a <br />flnge servtce rams <br />Number of physicians <br />reached <br />Number of pharmacies <br />distributing naloxone <br />Number of pharmacies <br />distributing over the <br />counter naloxone <br />Number of naloxone kits <br />distributed through syringe <br />exchange <br />a <br />a <br />a <br />a
