Attachment C <br />7.4.4 RentalAgreements <br />Shelter Programs must ensure one of the following types of agreements are in place if move-in <br />assistance is paid on behalf of a participant: lntent to Rent, Lease or Certification of Payment <br />Obligation. <br />7.4,4,L lntentto Rent <br />At a minimum, an lntent to Rent form must contain the following/ Name of tenant,/ Name of landlord <br />/ Address of rental property,/ Rent rate,/ Signature of landlord/date <br />7,4,4.2 Lease <br />At a minimum, the lease or rental agreement between the participant and the landlord must contain <br />the following:,/ Name of tenant,/ Name of landlord,/ Address of rental property <br />'/ Occupancy (who gets to live at the rental),/ Term of agreement (lease start and end date),/ Rent rate and date due./ Deposits (if any and what for/term),/ Signature of tenant/date,/ Signature of landlord/date <br />7,4,4,3 Certification of Payment Obligation <br />A Shelter Program Certification of Payment Obligation Form is required for rent subsidies paid to a <br />friend or family member who is not in the business of property management. <br />Page 045 <br />28
