3. County Priorities: Indicate which of the following priorities are being addressed by <br />the proposed project. {Select all that apply and give a brief 100 -word description for <br />each selected.) <br />✓ Increase capacity and sustainability for homeless sheltering. <br />The Ten -Year Plan identified subsidized property unit wait lists as exceeding two years. The Ten - <br />Year plan further identified subsidized senior housing as being at the top of the list of needs to <br />be addressed. The increased assessment, advocacy, and case management hours will greatly <br />increase the ability to move seniors on fixed incomes into market units, units based solely on <br />income, or permanent subsidized housing and eventually out of the SSAP unit freeing the units <br />up for others on the wait lists. HopeSource utilizes Briarwood's common area for senior <br />education on life skills and information on community resources, and the Polaris project in <br />Upper County also accommodates homeless seniors with rent and wrap-around case <br />management. These two examples of how to successfully bridge county, city and non-profit <br />homeless housing funds increase the probability of other projects being launched and <br />encourages landlords to enter as housing providers for this group increasing the sustainability <br />of homeless housing options. <br />❑ Connect shelter clients to services to reduce homelessness. <br />✓ Increase site-based rental vouchers for people who are homeless with <br />wrap-around case management through current partnerships. <br />Because of lack of units in the community, frequently new senior clients who are literally or at <br />risk of homelessness are placed at our senior partner site, Briarwood, in either subsidized or <br />non -subsidized units. Briarwood is the premier housing for able-bodied seniors on fixed <br />incomes. The dedication of these SSAP funds for direct services will increase the number of <br />available site-based rental vouchers and will be combined with wrap-around case management <br />to ensure housing is sustained. <br />❑ New affordable housing development. <br />✓ Increased access to rental vouchers. <br />N <br />
