6.Sustainability:Describeyourability,experience,andresourcestoimplementandsustaintheprojectduringthetermoftheproject.(250wordsmax.)TheSSAPprogramisnotsustainablewithoutKittitasCountyfunds.ThisprogramisuniquetoKittitasCountyandtherearenootheravailablefundingsourcesthatmeettheseexactcriteria.Thecountyhasconsistentlyandrightlysaidthatitisvitaltoensurethatnosenior(65orolder)inKittitasCountysuffersfromhousinginsecurity,thattheymaintaintheirintegrityandsenseofhonorinatimeofsignificantneed,andthatourcommunityprotectsandcaresforitsvaluedseniormembers.HopeSourcecontinuestoseeksustainablefundingsourcesforthisinvaluableprogramsuchastheROEFoundationgrantforseniorsandisindiscussionwithfoundationsandotherStateandFederalgrants.HopeSourceutilizesothergrantsourcessuchastheConsolidatedHomelessGranttobackfillthecountyfundingandsponsorsannualfundraiserstoaugmentgovernmentandfoundationgrants.HopeSourceSeniorAdvocatesaretrainedinfamilydynamics,homehealthworkercertification,carecoordination,mentalfirstaid,SOARsbenefitstraining,de-escalationandtrauma-informedcare.Advocateshaveacombined38yearsofexperienceworkingwithvulnerablepopulationsandseniors.HopeSourcehasworkedwithALTCandMedicaidforovertenyearsservingseniors.HopeSourcefinancialcontrolsincludeadherencetoFiscalPoliciessupportedbyOMBCircularrequirementsandaccountingpractices.HopeSourcepaysattentiontoeveryopportunitytodecreasecostssothatfundinggoesdirectlytoservingvulnerablehouseholds.HopeSourceisannuallyauditedbyanexternalaccountandhasreceived12consecutiveyearsofcleanaudits.10
