3.4.10.1 Submit final billing for services provided within forth -five (45) <br />days after the end of the State Fiscal Year (June 30). <br />3.4.10.2 Submit final billing for services for SOR and PFS within forty-five <br />(45) days after the end of each Federal Fiscal Year (September <br />29). <br />3.4.10.3 Submit final billing for services for STR within forty-five (45) days <br />after the end of the fiscal period for STR (April 30). <br />3.4.11 The Contractor shall ensure all expenditures for services and activities under <br />the Agreement are submitted on the A-19 invoice appropriate for Minerva data <br />entry. In the event the Contractor or a subcontractor fails to maintain reporting <br />obligations under this Agreement, HCA reserves the right to withhold <br />reimbursements to the Contractor until the obligations are met. <br />3.4.12 Administrative costs shall be billed separately from direct prevention services <br />as indicated on the A-19 invoice. <br />3.4.12.1 Administrative costs are defined in the Fiscal/Program <br />Requirements. <br />3.4.12.2 The Contractor shall use no more than eight percent 8% of the <br />SOR, PFS, STR, and/or DMA allocation for administrative costs. <br />3.4.12.3 No SABG funds allocated in this contract shall be used for <br />administrative costs. Admin for SABG is allocated as GFS. <br />3.4.13 HCA reserves the right to reduce the Prevention funds awarded in the <br />Agreement if the Contractor expenditures are below 60% of expected levels <br />during the fiscal quarter. Expenditures will be reviewed quarterly. <br />3.4.14 SOR, PFS, STR, and DMA funds may not be carried forward from year toyear, <br />based upon their respective fiscal year. <br />3.4.15 Based upon Exhibit D, Awards and Revenue (A&R), the source of funds in this <br />contract may include, as applicable, Substance Abuse Block Grant (SABG) <br />CFDA 94.959, the Washington State Dedicated Marijuana Account Fund <br />(DMA), General Fund State (GFS), the State Opioid Response (SOR) Grant <br />CFDA 93.788, the State Targeted Response (STR) Grant CFDA 93.788, and/or <br />the Partnerships for Success (PFS) Grant CFDA 93.243. <br />3.5 CONTRACTOR AND HCA AGREEMENT MANAGERS <br />3.5.1 Contractor's Agreement Manager or designee will have prime responsibility <br />and final authority for the services provided under this Agreement and be the <br />principal point of contact for the HCA Agreement Manager for all business <br />matters, performance matters, and administrative activities. <br />Washington State Page 14 HCA Contract No. K3924 <br />Health Care Authority <br />
