The Department is the Recipient and Pass-through Entity of the 15PDM Award EMS-2016-PC-0005, which with each of its attachments is incorporated in and attached hereto as Attachment 6, and has made a subaward of Federal award funds to the SUBRECIPIENT pursuant to this . nt.
