Partnerships For Success (PFS) <br />Community Coalition Mentoring Project <br />Year 5 September 30, 2017 to September 29, 2018 <br />1. Purpose. <br />Exhibit H <br />This purpose of the project is to increase the capacity for coalitions to mentor another coalition. The <br />community mentoring project should be focused on providing another coalition with technical <br />assistance and guidance on the topics related to prevention theory, Strategic Prevention Framework, <br />and implementation of approved prevention programs. <br />2. Statement of Work. <br />The Contractor shall provide the services and staff, and otherwise do all things necessary for or <br />incidental to the performance of work, as set forth below: <br />Coalition Mentoring Incentive Project: <br />Coalition coordinator and chairperson visit at least one (1) peer coalition (by request or recommended <br />by DBHR) during that other coalition's monthly full -membership coalition meeting for the purpose of <br />cross -site sharing. <br />a. Spend four (4) hours each month mentoring another coalition. <br />Each meeting should be focused on providing another coalition with technical assistance and <br />guidance on the topics related to prevention theory, Strategic Prevention Framework, and <br />implementation of approved prevention programs. <br />Meetings may be in-person or conducted virtually using webinar technology. A phone conference <br />will not be sufficient. <br />b. Coalition coordinator and chairperson shall visit at least one (1) mentee coalition meeting during <br />that other coalition's monthly full -membership coalition meeting for the purpose of providing <br />technical assistance. <br />(1) Submit coalition mentor meeting dates and agenda to DBHR at least five (5) days before the <br />meeting is to occur. <br />(2) Submit final report on how mentor activities have increased the capacity and coalition activities <br />for both the mentor coalition and the coalition mentee on June 23, 2018. <br />(3) Attend at least one (1) peer coalition meeting (on-site) to visit and share experiences and submit <br />dates and meeting agenda to PSM. <br />3. Reporting. <br />Enter each mentoring activities as a new "Log" under the Coalition Program in the Minerva. <br />Report all project efforts into the Minerva by the 15th of each month following month of service. <br />DSHS Central Contract Services <br />1644CS Prevention Services - County (6-26-2015) Page 49 <br />
