(2) Contract directly for expert consultation from Community Systems Group for a minimum of 10 <br />hours during contract period to include: <br />(a) Consultation and technical assistance for uploading the coalition's logic model into the CCB. <br />(b) Designing and maintain the Coalition's CCB page for specific data entry related to the logic <br />model activities. <br />(c) Training and consultation to become Primary Observers to identify the local activities related <br />to the coalition's logic model and strategies in the coalition's action plan that should be <br />coded and entered into the ODSS. <br />(d) Quarterly calibration scoring calls between Primary Observer(s), DSHS Reliability Scorer, <br />and CSG Calibration Scorer. <br />4. Reporting. <br />The Contractor Primary Observers shall: <br />a. Enter data monthly into the CCB System by the 15th of the month for previous month's services. <br />(1) Report quarterly a minimum of 40 complete data entries (20 Community Accomplishments and <br />20 Capacity Building Accomplishments) into the Community Check Box Evaluation System <br />(CCB) using the current Community Check Box Evaluation System Codebook that reflect the <br />work of the coalition for the previous month. A target of 100 complete data entries is strongly <br />encouraged to enhance use of the CCB system. <br />(2) Complete entries into the CCB are defined as entries per Community Accomplishment and <br />Capacity Building Accomplishment which critically reflect the impact made in the community by <br />the respective Coalition. Entries into the CCB will demonstrate the Coalition's efforts toward <br />accountability, self-evaluation, continuous improvement, and ongoing monitoring of progress <br />against the work identified in the Coalition's Action Plan. Outputs from the CCB should be used <br />in analysis of Coalition efforts and used in the Strategic Planning process, as well as a parallel <br />compliment to the outputs from the Minerva MIS Reporting System. <br />b. Participate and present in annual DBHR Summer Institute Community Surveillance Pilot Project <br />Presentation in June 2018. <br />c. Complete and submit two supplemental DBHR CCB Evaluation Mini -Reports (due November 30, <br />2017 and April 30, 2018) on the coalition's use of the CCB data outputs. This shall be completed by <br />the Coalition Coordinator(s) and the acting Coalition Member(s) who have implemented the CCB <br />Community Surveillance Pilot Project. <br />5. Performance Work Statement. <br />Due to the nature of this project's potential contribution to statewide prevention system development, if <br />the Contractor shall fail to report services in the CCB data collection software application by the 1511 of <br />the month for two or more consecutive months during the contract period, the resulting penalty may be <br />exclusion from future opportunities to continue participation in the Community Surveillance Pilot <br />Targeted Enhancement Project. <br />6. Consideration.
