ACTION REQUESTED:
The Board of County Commissioners approved the signature of the Public Health Director on the
Consolidated Contract Amendment 12. (Effective 01/01/2025-12/31/2027)
BACKGROUND:
The Consolidated Contract Amendment 12 is amended by the following:
Adds Statements of Work for the following programs:
Infectious Disease Syndemic Prevention Services-SSP - Effective July 1, 2026
Office of Resiliency & Health Security-PHEP - Effective July 1, 2026
Amends Statements of Work for the following programs:
Maternal & Child Health Block Grant – Effective January 1, 2025
Deletes Statements of Work for the following programs:
Exhibit B-12 Allocations, attached and incorporated by this reference, amends and replaces
Exhibit B-11 Allocations as follows:
Increase of $93,991 for a revised maximum consideration of $6,356,942.

Kittitas County Auditor’s Office: Reviewed and approved: August 2026
RECOMMENDATION:
The Board of County Commissioners approved the signature of the Public Health Director on the
