Contract/Grant Summary: This MOU is to allow for Crisis Connections to provide 24/7 access to a scheduling
phone line to making "bridging appointments" for patients who are discharging from a Correctional Institution in
the context of opiate use concerns. Three options of outpatient providers in their area will be offered with at least
one telehealth option. KCJ will provide the individuals information, a brief synopsis, and confirmation of verbal
agreement of intent to attend appointment provided by CC. CC will provide clinic/provider information and
specifics appointment appoint to staff facilitating release to be given to the individual.
