Re: J Mod Modifications
Per your request, we are pleased to offer the following change order proposal #19 (COP 19).
If approved, Belsaas and Smith’s onsite team will make the requested upgrades to the J Mod
space. This will include:
•Installing a new porcelain wall-hung toilet in the restroom
•Cleaning and sealing the floor tile grout in the restroom
•Installing a new 3/0 x 7/0 18ga flush door and associated hardware
•Cutting and grinding the supports for the tables and seating and patching the flooring
smooth
•Installing the selected carpet squares throughout the entire room
•Making all the requested electrical modifications
•Painting all the walls and the new door to the restroom
Total cost of proposal: Forty-One Thousand Four Hundred Seventy-Nine Dollars $41,479.32

*Note: Door lead time is 8 weeks from change order approval unless a wood or spare door is
approved
