ATTACHMENT 12-D (1)

December 2022

Community Development Block Grant
CONTRACT CLOSE OUT REPORT – PUBLIC SERVICES
1. Contract Number: 22-62210-008
2. Grantee Jurisdiction: Kittitas County
3. Chief Administrative Official and Title: Laura Osiadacz, Chair, Board of County Commissioners
4. CDBG Contract End Date or Project Completion Date: June 30, 2025
5. Final Public Hearing Date: September 16, 2025
6. Brief project summary and results achieved:

The HopeU life skills program was designed to support individuals navigating housing instability,
unemployment, and other barriers by providing practical tools and compassionate guidance. Through
classes focused on financial literacy, housing readiness, employment preparation, and personal
development, participants gained not only essential life skills but also a renewed sense of agency.
Throughout the program, individuals shared stories of growth—learning how to create a budget for the
first time, understanding their rights as tenants, preparing for job interviews, and building confidence in
themselves and their future. Many used the program as a steppingstone, connecting with additional
services and making progress toward goals they once felt were out of reach.
HopeU serves as more than a series of workshops, it is a place where people feel seen, supported, and
capable of change. The program embodied HopeSource’s Whole Person Care approach by treating each
person as more than their circumstances and offering a pathway forward built on dignity and hope.

7. Service area of the project/activity(s): Life skills workshops and 1:1 coaching.
The chief administrative official of the grantee jurisdiction certifies that:
1. The financial information and data provided in this report is accurate as of the signature date indicated below.
2. The project was completed in accordance with the grant contract and scope of work.
3. A final public hearing was conducted to assess project performance.
4. Records supporting the information provided in this report will be maintained for a minimum of six (6) years
from final closeout and made available upon request.
