UILrH <br />FLtEjrsBuRq-FAMrLy MEplgtNE pBA COMPAp$ prREcr_ EBB THE KTTT|TA,S <br />GOUNTY JAIL <br />This Agreement, effective as of the date signed below, is made by and between Kittitas <br />County (heroinafter "the County") and Ellensburg Family Medicine DBA CompassDireot <br />("Contraclor"). The Gounty and Contractor agree as follows: <br />That the existing Agreement, previously entered between the parties on Decernber 7tlt, <br />2021, shall be modified as allowed by section 1Z (twelve) of that agreement. The <br />purpose of this Modification is to allow for additlonal compensation for adminlstrative <br />fees, compensation for malpractice coverage cost increa$e, and additional nursing <br />hours. The total cost to the Contractor will increase from no greater than $29S,000 to no <br />greater than $290,000. This wlll be paid from the same fund. <br />$cope of work: <br />1. Distribute mediclne to the inmates at their designated times <br />2. Chart review and audit <br />3. Trlage of any acute needs <br />4. Rocords requosts and reviews <br />5. Assistance with implementation of the elestronic medical records system <br />6. Provide in-facility lab draws (also done by other staff from their office) <br />i, Those services can be done in the Jall wlth no need to transport <br />inmate to KVH <br />ii, Pricing is substantially lower than KVH ($ee attached price <br />comparlson) <br />7. Provide in-facility X-ray services (also done by other stafffrom their office) <br />i. Contractor will contract wlth FASTEX, a local x ray company out of <br />Yaklma <br />ll, FA$TEX can perform images in the jail with no need to transport <br />inmate to KVIj <br />iii. Priolng is substantially lower than KVH imaging center ($ee <br />attached prlce comparison) <br />Compensalion: <br />1. Provide nursing serulces at the jailan average of 40 hours/week. <br />A.
