1. Applicant Profile and Qualifications: Tell us about your organization’s qualifications.
This includes the purpose of your work, goals, relevant history, and experience working
in mental health/chemical dependency services, experience managing public funds, and
any other relevant information. (250 words) 10 points possible
2. Project Description Summary: Provide a brief summary of the project being proposed
and a general description of how it will address mental health and/or chemical
dependency in Kittitas County, including your project’s anticipated timeline and
deliverables. (250 words) 15 points possible
3. County Priorities: Which priority population does the proposed project apply to? (Select
all that apply and give a brief 100-word description for each selected.) 15 points possible
Behavioral health disorders prevention, treatment, and/or recovery for:
☐ Youth
☐ Veterans
☐ The justice involved
☐ Those at high risk of suicide
4. Partnerships: List your key community partnerships for the proposed project and what
their role will be. (250 words) 5 points possible
5. Gaps and Duplication in Services: Describe how your proposed project fills a gap in
mental health and/or chemical dependency services in Kittitas County. Include a
description of how the project is or is not a duplication of current services. (250 words)

6. Goals and Objectives: Fill in the Goals and Objectives Table with at least two SMART
goals for your project. Refer to the example and fill out as many forms as needed.

types of financial support that will be used to complete the project. Please include both
guaranteed support and support that may still be uncertain. (250 words) 5 points

3. Sustainability: Describe how the project will build sustainability to operate a basic level
of service beyond the funding period. (250 words) 10 points possible
4. Partial Funding: If this project were to receive only partial funding from this source, how
would the project be impacted? Can the proposal be adjusted for less than the
requested funding amount? (250 words) 5 points possible
