SMART GOALS AND OBJECTIVES <br />• Outline at least two SMART Goals for your project. SMART Goals are Specific, Measurable, <br />Achievable, Relevant, and Time-bound (achieved within a certain timeframe). <br />• Please complete the “Successful Benchmarks” table at the bottom of the document. Grantees <br />will be required to report on these benchmarks. <br />EXAMPLE PAGE <br />SMART GOAL: Expose 25% of Kittitas County high school students to a 45-minute suicide prevention <br />curriculum by December 2024. <br /> Measures (How will you know you are making progress toward your goal?) Data Source <br />Number of attendees at suicide prevention presentations Internal tracking sheet <br /> Strategy #1 <br /> <br />Work with community partners to create a youth suicide <br />prevention curriculum. <br />Measurable Objectives: <br />• Hire a Program Coordinator in the first six months. <br />• Convene a stakeholder group to give input into the design <br />of an evidence-based or promising curriculum. <br />• Create a booklet that will be given to program participants. <br />Strategy #2 <br /> Build relationships with every high school in Kittitas County. <br />Measurable Objectives: <br />• Connect with high school principals to draft partnership <br />agreements. <br />• Schedule classroom time with teachers, ensuring that a <br />mental health counselor is available for follow-up referrals <br />obtained during the presentation. <br />Strategy #3 <br /> <br />Increase awareness of suicide prevention within the cultures <br />of Kittitas County high schools. <br />Measurable Objectives: <br />• Distribute educational materials and posters about <br />resilience including protective factors and positive coping <br />skills. <br />• Train teachers and other school staff to recognize the <br />warnings signs of suicide and to take action to connect <br />those students with counseling and other life-saving <br />resources. <br /> <br /> <br />SUCCESSFUL BENCHMARKS <br />NAME ONE ACTIVITY YOU WILL ACCOMPLISH FOR EACH REPORTING TIME INTERVAL <br /> <br />6 MONTHS <br /> <br />Hire a full-time Project Coordinator. <br /> <br />12 MONTHS <br /> <br />Begin giving presentations in high school classrooms. <br /> <br />18 MONTHS <br /> <br />Have 10 volunteer community members trained to give the presentation. <br /> <br />24 MONTHS <br /> <br />Compile evaluation data into a final report to the community (video slideshow).
