ACTION REQUESTED:
The Board of County Commissioners approves the signature of the Public Health Director on the Consolidated Contract Amendment 11. (Effective Dates: January 1, 2022, through December
31, 2024)
BACKGROUND:
The Consolidated Contract Amendment 11 amends the following Statement of Work:
DCHS - ELC COVID-19 Response - Effective January 1, 2022
Office of Drinking Water Group B Programs - Effective January 1, 2022
Office of Immunization COVID-19 Vaccine - Effective January 1, 2022
Exhibit B-10 Allocations, attached and incorporated by this reference, amends, and replaced Exhibit B-8 Allocation as follows:
Increase of $25,878.00 for a revised maximum consideration for $3,732,244.00

Kittitas County Prosecutor’s Office: Reviewed and approved January 6, 2023
Kittitas County Auditor’s Office:Reviewed and approved on January 11, 2023
