This Contract betvraeen the State of Washington Department of Children, Youth & Families (DCYF) and the
Contractor is hereby amended as follows:
1. Purpose. The purpose of this Amendment is to update the Statementof Workto include Education
and Employment Training.
SpecialTerms and Conditions are amended as follovvs:
2. Section 1, Definitions add Subsection c. to read as follows:
c. 'EET' means Education and Employment Training.
Statementof Work-Exhibit B is amended as follows:
3. Section 1. Subsection b. Service Delivery is added to read as follovrs:
b. The Contractor shall provide statewide program consultation and development forthe EET program.
Services shall include, but are not limited to:
(1) Providing group and individualconsultationto Education and Employment Specialists (EES)
throughout the state;
(2) Developing, participating, and providing EET training;
(3) Monitoring by direct observation, video, or audio recordings of Job Readiness Training
sessions, and individual EES sessions for ad herence and compliance to EET prograri
standards;
(4) Reporting monitoring results to the JR EET Quality Assurance Specialist on a monthly basis;
(5) Providing assistance with individual EES improvement plans; and
(6) Participating in implementation and ongoing program developmentmeetings.
4. section 2. subsection a. Deliverabtes is amended to read as follows:
a. Monthly Reporting
The Contractor shall provide monthly activity and monitoring summary reporb (i-ACT and EET) tothe JR Juvenile Court program Administrator.
All other terms and conditions of this Contract remain in full force and effect.
