Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />August 2, 2022 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Public Health Director on the <br />Consolidated Contract Amendment 5 (Effected dates:1/1/2022 – 12/31/2024). <br /> <br />BACKGROUND: <br />Adds Statements of Work for the following programs: <br />• Covid Mass Vaccination -FEMA-Effective January 1, 2022 <br />• DCHS-ELC COVID-19 Response-effective January 1, 2022 <br /> <br />The amendment also adds an increase of $217,313.00.00 for a revised maximum consideration <br />of $3,095,724.00. <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved on 7/8/2022 <br />Kittitas County Auditor’s Office: Reviewed and approved on 7/18/2022 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Director on the <br />Consolidated Contract Amendment 5 (Effected dates:1/1/2022 – 12/31/2024). <br /> <br />HANDLING: <br /> <br /> <br />LEAD STAFF: <br />Chelsey Loeffers
