PROFESSIONAL SE AGREEMENT <br />This Agreement is made by and between Kittitas County on behalf of Kittitas County <br />Superior Court, Juvenile Court Seruices, (hereinafter "the County") and Kristi Hunzikei, <br />MSW, LICSW, CSOTP (hereinafter "Contractor"). The County and Contractor agree as <br />follows: <br />General Conditions; Exhibit A (Scope of Work); Exhibit B (Compensation); <br />Exhibit C (Proof of lnsurance). <br />copies of which are attached hereto and incorporated herein by this reference as fully as if <br />set forth herein. <br />The term of this Agreement shall commence on the Septembe r 1 ,2022 and continue <br />until August 31, 2023 Any party may terminate this Agreement by giving thirty (30) days'notice in writing either personally delivered or mailed postage-piepaid by certitied miil, <br />return receipt requested, to the pafiy's last known address for the purposes-of giving notice <br />under this paragraph. <br />Contractor acknowledges and by signing this Agreement agrees that thelndemnification provisions set forth in Paragraphs 7 (lndependent Contracto4, g (Taxes),15 (Defense and lndemnity Agreement), 21 (PatenUCopyright lnfringement) and Zl.+(Confidentiality), are totally and fully part of this Agreement and have been mutuallynegotiated by the parties. <br />Professional Services Agreement (rev . 0g 124 t201 g) <br />Page 1 of 18
