Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />May 17th, 2022 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves and signs the resolution approving the signature <br />of the Public Health Director on the Consolidated Contract Amendment 4 (effected dates <br />1/1/2022 – 12/31/2024). <br /> <br />BACKGROUND: <br />Adds Statements of Work for the following programs: <br />o COVID-19 Mass Vaccination-FEMA - Effective January 1, 2022 <br />o DCHS-ELC COVID-19 Response - Effective January 1, 2022 <br />o LSPAN-Local Strategies for Physical Activity & Nutrition - Effective January 1, <br />2022 <br />o Maternal & Child Health Block Grant - Effective January 1, 2022 <br />o Office of Drinking Water Group A Program - Effective January 1, 2022 <br />o Office of Immunization COVID-19 Vaccine - Effective January 1, 2022 <br />o Supplemental Nutrition Assistance Program-Education - Effective January 1, 2022 <br /> <br />The amendment also adds an increase of $254,119.00 for a revised maximum consideration of <br />$2,848,411.00. <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved on 5/4/2022 <br />Kittitas County Auditor’s Office: Reviewed and approved on 5/4/2022 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves and signs the resolution approving the signature <br />of the Public Health Director on the Consolidated Contract Amendment 4 (effected dates <br />1/1/2022 – 12/31/2024). <br /> <br />HANDLING: <br /> <br /> <br />LEAD STAFF: <br />Tristen Lamb <br />Chelsey Loeffers
