Professional Services Agreement Amendment <br /> <br />PROFESSIONAL SERVICES AGREEMENT AMENDMENT <br /> <br /> This contract amendment is made by and between Kittitas County (hereinafter referred to <br />as “County”) and FISH (hereinafter referred to as “Contractor”). The County and the Contractor <br />agree as follows: <br /> <br />WHEREAS, Kittitas County and Contractor entered into a professional services contract in <br />August 2021; and <br />WHEREAS, the agreement was created with the ability to be renewed for additional periods <br />after the contract end date of February 1, 2022; and <br />WHEREAS, the Kittitas County Health Network will not be able to provide grocery resources <br />for the full length of this contract period to individuals in COVID-19 isolation; and <br />WHEREAS, due to the continued high case rates of COVID-19 in Kittitas County, there is a <br />continued need to make isolation and quarantine meals available to food insecure <br />individuals isolating outside of their homes; and <br />WHEREAS, both parties desire to continue the agreement, with the following changes: <br />Contract end date to be extended to August 1, 2022, with an option for renewal by <br />amendment if agreed upon by both parties. <br /> <br />NOW THEREFORE, BE IT RESOLVED that the Board of County Commissioners of Kittitas <br />County, Washington, authorizes execution of the contract amendment providing <br />for the updates incorporated herein by this reference. <br /> <br />DATED this __________ day of ____________________, 2022, at Ellensburg, Washington. <br /> <br /> APPROVED: <br /> <br />HOPESOURCE BOARD OF COUNTY COMMISSIONERS <br /> KITTITAS COUNTY, WASHINGTON <br /> <br /> <br />_______________________ __________________________________________ <br />Signature of Signatory Chair <br />Date___________________ <br /> <br /> __________________________________________ <br /> Vice-Chair <br /> <br /> <br />_______________________ ____________________________________ <br />Print Name of Signatory Commissioner
