ACTION REQUESTED:
The Board of County Commissioners authorizes and signs the Professional Service Agreement
Amendment 1 between Kittitas County Public Health and Community Health of Central
Washington. (Effective dates: October 1, 2021-September 30, 2022)
BACKGROUND:
This is an amendment to the agreement with Community Health of Central Washington and the Kittitas
County Public Health Department to provide services for the Children and Youth with Special Health
Care Needs program, part of the Maternal and Child Health Block Grant. This amendment increases the
total agreement amount and timeframe to include the entire program year now that KCPHD has
received the updated Consolidated Contract from the Department of Health. No change in total amount
from last year.

Kittitas County Auditor’s Office: Reviewed and approved on 01/31/2022
RECOMMENDATION:
The Board of County Commissioners authorizes and signs the Professional Service Agreement
