PROFESSIONAL SERVICES AGREEMENT <br />- -This AgreelFl!is mlde by and between Kittitas County (hereinafter "the County,')and CONTRACTOR FISH Community Food Bank (hereinaftei "bontractof'). The Countyand Contractor agree as follows: <br />General Conditions; Exhibit A (Scope of Work); Exhibit B (Compensation); <br />Exhibit C (Proof of lnsurance); Exhibit D (Cost Breakdown Worksheet) ' <br />copies of which are attached hereto and incorporated herein by this reference as fully as ifset forth herein. <br />The term of this Agreement shallcommence on the 1st of August,2021, and continuefor a period of six months until Febru ary 1 , 2O22 or until such timJ that the Kittitas CountyHealth Network (KCHN) receives funding and is in a position to takeoversuch services. ThisA-greement is subject to renewal for additional successive periods upon the written consentof the Parties at the end of the six month period, if the Kittitas Coung Health Network isunable to takeover services.
