<br />Professional Services Agreement Amendment 1 <br />509 Facility Services <br />Page 1 of 2 <br /> <br /> <br /> <br /> <br /> <br /> <br />COVID-19 ISOLATION & QUARANTINE CLEANING AND DECONTAMINATION <br />PROFESSIONAL SERVICES AGREEMENT AMENDMENT 1 <br /> <br /> <br /> This contract amendment is made by and between Kittitas County (hereinafter “the County”) <br />and 509 Facility Services, Inc. (hereinafter “Contractor”). The County and Contractor agree as <br />follows: <br /> <br />WHEREAS, Kittitas County and Contractor entered into a professional services agreement in <br />December of 2020; and <br /> <br />WHEREAS, section 12 of the agreement allows for modifications that are made in writing and <br />signed by both the parties; and <br /> <br />WHEREAS, both the County and the Contractor desire to amend the agreement to extend the <br />term of agreement until October 1, 2022 and allow for additional one year renewal <br />periods; and <br /> <br /> WHEREAS, both the County and the Contractor desire to amend the agreement to update the <br />invoice contact person to Katie Odiaga at Kittitas County Public Health, <br />katie.odiata@co.kittitas.wa.us; and <br /> <br />WHEREAS, the rest of the agreement remains unchanged and in full effect. <br /> <br />NOW THEREFORE, in mutual consideration of the terms and conditions hereinafter set forth <br />and those contained in the agreement, it is hereby agreed as follows: <br />1) The term of the agreement is extended until October 1, 2022, with the option to renew <br />for additional one-year periods upon written agreement of the parties. <br /> <br />2) Exhibit B Section a, shall now read: Contractor shall submit invoices twice per month <br />for billing periods of the 1st through 15th and 16th through the 31st. Invoices may be <br />emailed to Katie Odiaga at Kittitas County Public Health, <br />katie.odiata@co.kittitas.wa.us. <br />
