COVID-19 ISOLATION & QUARANTINE CLEANING AND DECONTAMINATION
PROFESSIONAL SERVICES AGREEMENT AMENDMENT 1
This contract amendment is made by and between Kittitas County (hereinafter “the County”)
and 509 Facility Services, Inc. (hereinafter “Contractor”). The County and Contractor agree as
follows:
WHEREAS, Kittitas County and Contractor entered into a professional services agreement in
December of 2020; and
WHEREAS, section 12 of the agreement allows for modifications that are made in writing and
signed by both the parties; and
WHEREAS, both the County and the Contractor desire to amend the agreement to extend the
term of agreement until October 1, 2022 and allow for additional one year renewal
periods; and
WHEREAS, both the County and the Contractor desire to amend the agreement to update the
invoice contact person to Katie Odiaga at Kittitas County Public Health,
katie.odiata@co.kittitas.wa.us; and
WHEREAS, the rest of the agreement remains unchanged and in full effect.
NOW THEREFORE, in mutual consideration of the terms and conditions hereinafter set forth
and those contained in the agreement, it is hereby agreed as follows:
1) The term of the agreement is extended until October 1, 2022, with the option to renew
for additional one-year periods upon written agreement of the parties.
2) Exhibit B Section a, shall now read: Contractor shall submit invoices twice per month
for billing periods of the 1st through 15th and 16th through the 31st. Invoices may be
emailed to Katie Odiaga at Kittitas County Public Health,
katie.odiata@co.kittitas.wa.us.
