ACTION REQUESTED:
The Board of County Commissioners approves the signature of the Public Health Director on the
Consolidated Contract Amendment 22 (effected dates 1/1/2018 – 12/31/2021).
BACKGROUND:
The Consolidated Contract is the vehicle used by the Washington State Department of Health to
channel state authorized funding for programs and services to the Kittitas County Public Health
Department. The Amended Consolidated Contract amends the following:
Adds a statement of work for the following programs:
• Emergency Preparedness and Response PHEP- Effective July 1, 2021
• Foundational Public Health Services-Effective July 1, 2021
Amends Statements of Work for the following programs
• COVID 19 Coordinated Response- Effective July 1, 2020
• LSPAN- Local Strategies for Physical Activity and Nutrition- Effective February 19, 2019
• Maternal and Child Health Block Grant- Effective January 1, 2018
• Office of Drinking Water Group A Program-Effective January 1, 2018
Exhibit B-22 Allocations
• Increase of $182,929.00 for a revised maximum consideration of $3,779,101.00

Kittitas County Auditor’s Office: Reviewed and approved 09/28/2021
RECOMMENDATION:
The Board of County Commissioners approves the signature of the Public Health Director on the
Consolidated Contract Amendment 22 (effected dates 1/1/2018 – 12/31/2021).
