PROGRAM
This Agreement, effective as of the date signed below, is made by and between Kittitas
County (hereinafter "the County") and Kittitas County Health Network ("Contractor"). The
County and Contractor agree as follows:
That the existing Agreement for the above referenced M.A.T. program, ("Agreement")
previously entered between the parties on July 6th, 2021, shall be modified as allowed
by section 12 (twelve) of that agreement. The purpose of this Modification is to allow for
change in required credentials of the Nurse Care Manager position due to unexpected
inability to recruit and hire a License Practical Nurse as had been anticipated by the
parties. The parties agree herein that the Nurse Care Manager position may filled by
any category of Nurse as defined in RCW 18.79 who is admitted to such practice in
accordance with that chapter. It is further agreed that the financial provisions of the
existing agreement will be adjusted equitably pursuant to section 12.3, but that such
adjustment will involve only internal allocation and will not change the total amount of
compensation.
All terms, conditions, attachments, documents and exhibits, including the term of the
agreement as described in the original Agreement are incorporated herein by this
reference as fully as if set forth herein.
