ACTION REQUESTED:
The Board of County Commissioners approves and signs the Amendment to the Professional
Service Agreement between Kittitas County Health Department and Kittitas County Health
Network. (Effective dates 04/26/2021 – 04/26/2022)
BACKGROUND:
The PSA between Kittitas County and Kittitas County Health Network is established to identify
health impacts, health needs, and gaps in services. KCHN shall support the creation, facilitations,
and evaluation of surveys, interviews, and focus groups to hear from community members how
their lives have been impacted over the past year due to COVID-19. The amendment extends the
end date of the contract.

Kittitas County Prosecutor’s Office: Reviewed and approved 08/08/2021
Kittitas County Auditors’ Office: Reviewed and approved 08/10/2021
RECOMMENDATION:
The Board of County Commissioners approves and signs the Amendment to the Professional
Service Agreement between Kittitas County Health Department and Kittitas County Health
Network. (Effective dates 04/26/2021 – 04/26/2022)
