Not to exceed $186,118.67; 2023 amount: not to <br />exceed $186,118.67 <br /> <br />Grant/Contract Number: SH21-016 <br /> <br />Contract/Grant Summary: Kittitas County Health Network will provide Medication Assisted Treatment <br />(MAT), Substance Use Disorder (SUD) counseling and treatment services, and mental health counseling <br />to individuals both while they are in the Kittitas County Jail as well as upon release. KCHN staff will lead <br />this work through administration of subcontracts, tracking and reporting of data related to performance <br />and outcomes, convening the services providers for continuous quality improvement efforts, and <br />supporting post-release care coordination efforts. MAT services for individuals in the jail and upon <br />release will be jointly provided by Comprehensive Healthcare, Community Health of Central <br />Washington, and Kittitas Valley Healthcare. Merit Resource Services will provide SUD assessments, <br />counseling, peer support services for discharge planning and post-release care coordination, and post- <br />release treatment services.
