AGENDA DATE: July 6, 2021
ACTION REQUESTED: Request to Acknowledge an Amendment to the Occupational
Medicine Services Agreement between KVH Workplace Health Medicine and Kittitas
County Sheriff's Office.
BACKGROUND: This Amendment, effective June 1, 2021, amends the Agreement
that was approved by the Board on April 7, 2021, between KVH Workplace Health
Medicine and Kittitas County Sheriffs Office.
The purpose of this Amendment is to ensure that the prices set in Exhibit "A" to the
original Agreement are corrected to reflect the correct pricing structure.
INTERACTION: Kittitas County Prosecuting Attorney's Office has reviewed and
approved this agreement.
RECOMMENDATION: Request to Acknowledge an Amendment to the Occupational
Medicine Services Agreement between KVH Workplace Health Medicine and Kittitas
County Sheriff's Office.
