GONTRACT AMENDMENT
The Professional Services Agreement by and between Kittitas County (hereinafter "the
County") and Robin Read (hereinafter "Contractor"). The County and Contractor is hereby
amended as follows:
. Scope of Work increased to 200 hours (Exhibit A).. Compensation increased to maximum of $1S,000.00 (Exhibit B).
The term of this Amendment shall begin on May 11th, 2021 and will continue through
December 31, 2021. Any party may terminate this Agreement by giving thirty (30) days'
notice in writing either personally delivered or mailed postage-prepaid by certitieO miit,
return receipt requested, to the party's last known address for the purposes-of giving notice
under this paragraph.
All other terms and conditions of the agreement remain in full force and effect.

SCOPE OF WORK
The Contractor shall provide up to 200 hours of assistance to the deparlment, to include
some of or all of the following areas:
a Public health emergency preparedness and response activities for COVID-19
This may include surveillance, epidemiology, laboratory capacity, infection
control, mitigation, communications, community engagement, and/or other
preparedness and response activities for COVID-19.
collect, organize, analyze, enter, and report covlD-19 data, utilizing CREST,
WDRS, and internal tracking sheets.
Work with partners to ensure testing is provided in a culturally and linguistically
responsive mannerwith an emphasis on making testing available to disproportionally
impacted communities and as part of the jurisdiction's contact tracing strategy.
Maintain a current list of entities providing COVID 19 testing and at what volume.
Work with partners to provide culturally and linguistically responsive community
outreach and education related to COVID-19.
Regional active monitoring activities, including partnerships with WA DOH, other
Local Health Jurisdictions (LHJs) in the region to collaboratively support
epidemiologic and surge capacity needs, and ensure regular communication among
LHJs in the region.

COMPENSATION
As full compensation for satisfactory performance of the work, the County shall pay
Contractor the following compensation:
$75.00 per hour, not to exceed $15,000.00.
