1. Contract expiration date August 15, 2021 remains unchanged.
a) On Contract Face Sheet, Box 6.
2. Contract performance period of July 1, 2020 through June 30, 2021 remains unchanged.
a) On Attachment A, Section VI - SPECIAL TERMS AND CONDITIONS, Section VI. PERFORMANCE PERIOD
AND PAYMENT.
b) On Attachment A, Section VII, B - THE COUNTY AGREES TO Use of Funding.
c) On Attachments C -F, dates under the page headings.
3. Change the overall FY21 funding amount from $316,236 to $342,736 an increase of $26,500.
a) On Contract Face Sheet, Box 2. $316,236 $342,736.
4. Replace the Original Budget Sheet (ATTACHMENT E) with the attached Revised Budget Sheet (ATTACHMENT
E), which amends the following:
a) Change the FY21 CPD6 funding amount $8 $26,500.
b) Unchanged the FY21 Basic Service Operating funding amount of $280,426.
