A Resolution to Cancel Payroll Fund Warrants
WHEREAS, warrant number 115658 and 116504 Payroll Fund was issued on August 5,
2019 and October 4, 2019, in the amount of $81.33 and $135.55 and,
WHEREAS, the warrant number 115658 and 116504, Payroll Fund was issued to Louis
Randal McDonald, the amount of $81.33 and $135.55, on August 5, 2019 and October 4, 2019 and
the paychecks were never cashed, and
WHEREAS, we have received an affidavit from Louis Randal McDonald to re-issue the
warrants.
NOW, THEREFORE BE IT RESOLVED warrant number 115658, dated August 5, 2019,
and warrant number 116504 dated October 4, 2019, Payroll Fund, payable to Louis Randal
McDonald in the amount of $81.33 and $135.551 be cancelled and re-issued from Accounts Payable
on December 28, 2020 to Louise Randal McDonald in the amount of $216.88.
