Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />November 5, 2019 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 10. (Effective date: 01/01/2018-12/31/2020) <br /> <br />BACKGROUND <br />The Consolidated Contract is the vehicle used by the Washington State Department of Health to <br />channel state authorized funding for programs and services to the Kittitas C ounty Public Health <br />Department. The amendment amends the following: <br />Exhibit A Statements of Work <br />Adds Statement of Work for the following: <br /> Foundational Public Health Services-Effective July 1, 2019 <br /> Office of Emergency Preparedness and Response-Effective July 1, 2019 <br />Amends Statements of Work for the following programs: <br /> LSPAN-Local Strategies for Physical Activity and Nutrition- Effective February 19, 2019 <br /> Maternal and Child Health Block Grant- Effective January 1, 2018 <br /> Office of Drinking Water Group A Program- Effective January 1, 2018 <br /> Office of Drinking Water Group B Program- Effective January 1, 2018 <br />Exhibit B-10 Allocations, amends and replaces Exhibit B-9 Allocations <br />Increase of $275,932 for a revised maximum consideration of $665,415.00 <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved on September 30, 2019 <br />Kittitas County Auditor’s Office: Reviewed and approved on October 14, 2019 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 10.
