LETTER OF AGREEMENT <br />Between <br />Kittitas County Health Network <br />And Kittitas County Public Health Department <br />For Services under the HRSA Opioid Response Planning Grant <br />Thank you for agreeing to provide services for the Health Resources and Services Administration (HRSA) <br />Opioid Response Planning Grant, administered by the Kittitas County Health Network (KCHN). This letter <br />summarizes the deliverables required by the Kittitas County Public Health Department (KCPHD) and to <br />be compensated for in the grant budget which runs from October 1, 2018 to September 30, 2019. <br />Definitions <br />The following definitions are key terms that will be used in this document and throughout the project in <br />question: <br />Project: <br />Sub-Contractor: <br />Grant: <br />Naloxone and Syringe Exchange Expansion <br />Kittitas County Public Health Department (KCPHD) <br />HRSA Opioid Response Planning Grant <br />Qualified KCPHD staff will be assigned to carry out the Services of this project. Tristen Lamb, <br />Administrator, Kittitas County Public Health Department, will be the primary contact for the project. <br />Proiect Scape of Work <br />KCPHD will improve access to harm reduction measures through strategic placement and expansion of <br />naloxone distribution and syringe exchange services. <br />Deliverables to be completed by September 30, 2019: <br />1. Provide weekly syringe exchange services in both Ellensburg and Cle Elum. <br />2. Provide opioid overdose response training upon request. <br />3. Distribute naloxone to individuals at risk of witnessing an opioid overdose. <br />4. Participate in the Opioid Response Work Group and associated deliverables. <br />5. Participate in the Community Prevention and Wellness Initiative addressing youth opioid <br />prevention at the Ellensburg School District. <br />6. Provide quarterly reports summarizing funded activities and deliverables including the following : <br />a. Number of opioid response trainings provided and number and type of participants <br />b. Number of naloxone kits distributed-first time, and refills <br />c. Number of syringe exchange participants referred to treatment for opioid use disorder <br />Schedule <br />The services described above in this agreement will commence on October 1, 2018, and be satisfactorily <br />completed on or before September 30, 2019. Total hours of work required and any costs incurred to <br />complete the project are the responsibility of KCPHD, and no costs will be compensated outside of those <br />included in the grant budget unless by prior written approval of KCHN. <br />Prop r ietary Rights <br />All materials produced as a result of this Agreement are to be public domain .
