ACTION REQUESTED:
The Board of County Commissioners approves the signature of the Public Health Administrator
on the Consolidated Contract Amendment 5. (Effective date: 01/01/2018-12/31/2020)
BACKGROUND
The Consolidated Contract is the vehicle used by the Washington State Department of Health to
channel state authorized funding for programs and services to the Kittitas County Public Health
Department. The amendment amends Statements of Work for the following program:
Office of Emergency Preparedness and Response-Effective July 1, 2018
Exhibit B-5 Allocations, attached and incorporated by this reference, amends and replaces
Exhibit B-4. Allocations as follow:
Increase of $885.00 for a revised maximum consideration of $269,554.00
INTERACTION:
Kittitas County Prosecutor’s Office: Reviewed and approved on November 14, 2018
Kittitas County Auditor’s Office: Reviewed and approved on November 21, 2018
RECOMMENDATION:
The Board of County Commissioners approves the signature of the Public Health Administrator
on the Consolidated Contract Amendment 5. (Effective date: 01/01/2018-12/31/2020)
