Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />October 16, 2018 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Kittitas County Public Health <br />Interim Administrator on the Consolidated Contract Amendment 4. (Effective Dates: January 1, <br />2018-December 31, 2020) <br /> <br />BACKGROUND: <br />The Consolidated Contract Amendment 4 adds a Statement of Work for the Office of Emergency <br />Preparedness and Response and Supplemental Nutrition Assistance Program Education. The <br />amendment also amends the Statements of Work for the Childhood Lead Poisoning Prevention <br />Program, Maternal and Child Health Block Grant, OICP-Promotion of immunization to improve <br />vaccination rates, Supplemental Nutrition Assistance Program. <br /> <br />INTERACTION: <br />Kittitas County Prosecuting Attorney’s Office: Reviewed and approved September 24 2018. <br />Kittitas County Auditor’s Office: Reviewed and approved September 25, 2018. <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Kittitas County Public Health <br />Interim Administrator on the Consolidated Contract Amendment 4.
