Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />August 7, 2018 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners’ authorizes the signature of the Administrator on the <br />Consolidated Contract Amendment 3. (Effective Date: January 1, 2018- December 31, 2020) <br /> <br />BACKGROUND: <br />The Consolidated Contract Amendment 3 adds Statements of Work for the FPHS Communicable <br />Disease and Support Capabilities and the OICP-Promotion of Immunizations to Improve <br />Vaccination Rates. It also amends statements of work for the Office of Drinking Water Group A <br />and B programs. The Consolidated Contract increases the contract amount by 57,600.00 for a <br />new revised maximum of $151,121.00. <br /> <br />INTERACTION: <br />Kittitas County Board of Health: N/A <br />Kittitas County Prosecutor’s Office: Reviewed and approved on July 20, 2018. <br />Kittitas County Auditor’s Office: Reviewed and approved on July 23, 2018. <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners’ authorizes the signature of the Administrator on the <br />Consolidated Contract Amendment 3.
