Central Washington ~ >.: b SeTVio::n Cl! <br />Family Medicine ~ommunllyHQOJjh <br />Residency Program of Control W0 1hln91on <br />Program Letter of Agreement <br />between <br />Central Washington Family Medicine Residency Program <br />and <br />Kittitas County Public Health Department <br />This Letter of Agreement is entered into by and between the Central Washington Family Medicine <br />Residency Program ("CWFMR") and Kittitas County Public Health Department ("Participating Site"). <br />This Letter of Agreement is effective for five years beginning November 21, 2018, unless terminated by <br />either CWFMR or Participating Site upon 18 months written notice, or upon mutual agreement. <br />1. Persons Responsible for Education and Supervision <br />The below mentioned people are responsible for the education and supervision of the residents while <br />rotating at Participating Site: <br />a. For CWFMR: Russell G. Maier, MD <br />b. For Participating Site: Mark W. Larson, MD <br />Other volunteer faculty who will supervise residents include: <br />Staff of Kittitas County Public Health Department <br />2. Responsibilities <br />The faculty at Participating Site must provide appropriate supervIsIon of residents in patient care <br />activities and maintain a learning environment conducive to educating the residents in the Accreditation <br />Council for Graduate Medical Education (ACGME) competency areas. The faculty must evaluate <br />resident performance in a timely manner during each rotation or similar educational assignment and <br />document this evaluation within four (4) weeks of the completion of the assignment. <br />1) CWFMR RESPONSIBILITIES <br />a. Background checks shall be required for all residents before being place at Participating Site . <br />Participating Site may refuse placement of a resident who does not provide the requested <br />records or who has a record of prior criminal conduct. <br />b. Residents will obtain and provide proof of any licensure requirements . <br />2. PARTICIPATING SITE'S RESPONSIBILITIES <br />a. Participating Site will provide residents with an educational experience within the scope of <br />Central Washington Family Medicine Residency Program <br />Program Letter of Agreement <br />Page 1 of 5
