DEPARTMENT OF <br />ECOLOGY <br />State of Washington <br />AMENDMENT NO.2 <br />TO AGREEMENT NO. W2RCPG-lS17-KiCPHD-00089 <br />BETWEEN <br />THE STATE OF WASHINGTON DEPARTMENT OF ECOLOGY <br />AND <br />KITTITAS COUNTY PUBLIC HEALTH <br />PURPOSE: To amend the above-referenced agreement (AGREEMENT) between the state of Washington Department <br />of Ecology (ECOLOGY) and Kittitas County Public Health (RECIPIENT) for the KCPHD Solid Waste Enforcement <br />(PROJECT). <br />The purpose of the amendment is to amend the scope of the work to allow for transferring $3,895.70 of the obligated <br />CPG budget from Task 3 (Unsecured Loads Community Outreach) to Task 1 (General Solid Waste Enforcement). <br />IT IS MUTUALLY AGREED that the AGREEMENT is amended as follows: <br />Project Short Description: <br />Original: <br />The RECIPIENT will monitor 8 active solid waste permitted facilities, 1 facility that is in post closure status, and 7 <br />exempt facilities for compliance; investigate solid waste related complaints; offer technical assistance regarding solid <br />waste issues; conduct secured load ordinance education events; as well as community syringe education and collection <br />events. The total eligible task cost for this Solid Waste Enforcement grant will be $88,882.67. <br />Amended: <br />The RECIPIENT will monitor 8 active solid waste permitted facilities, I facility that is in post closure status, and 7 <br />exempt facilities for compliance; investigate solid waste related complaints; offer technical assistance regarding solid <br />waste issues and community syringe education and collection events. The total eligible task cost for this Solid Waste <br />Enforcement grant will be $88,882.67. <br />CHANGES TO SCOPE OF WORK <br />Task Number: Task Cost: $49,693.97 <br />Task Title: Solid Waste Enforcement <br />Task Description: <br />The RECIPIENT intends to monitor solid waste handling facility compliance with applicable state solid waste <br />regulations. The RECIPIENT will monitor 8 active solid waste permitted facilities, I facility that is in post closure <br />status, and 7 exempt facilities for compliance; investigate solid waste related complaints; offer technical assistance <br />regarding solid waste issues; community syringe education and collection events. <br />Version 10/30/2015
