BOARD OF COUNTY COMMISSIONERS <br />COUNTY OF KITTITAS <br />STATE OF WASHINGTON <br />RESOLUTION <br />NO.2017-O:J i <br />RESOLUTION TO AUTHORIZE EXECUTION OF AN AMENDMENT TO A <br />PROFESSIONAL SERVICES AGREEMENT BETWEEN KITTITAS COUNTY AND <br />ELMVIEW <br />WHEREAS, Kittitas County, through the Kittitas County Public Health Department (KCPHD), <br />is charged with the preservation, promotion, and improvement of health in the <br />County; and <br />WHEREAS, KCPHD and Elmview entered into a professional services agreement dated, July 1, <br />2016, for the purpose of providing a coordinated and comprehensive local program <br />of services for persons with developmental disabilities; and <br />WHEREAS, the Agreement allows for amendments that are in writing and agreed to by both the <br />parties; and <br />WHEREAS, the Parties desire to amend the Agreement by increasing the payment rate <br />schedule, Attachment E, for Community Access, from $30.00 per service hour to <br />$32.00 per service hour as detailed in the amendment attached hereto and <br />incorporated herein by reference. <br />NOW THEREFORE, BE IT RESOLVED that the Board of County Commissioners of Kittitas <br />County, Washington, hereby authorizes the Kittitas County Public Health <br />Department to execute an amendment to a professional services agreement with <br />Elmview that is attached hereto, and incorporated herein by reference. <br />DATED this ]111 day of Eeb14A a~ 2017, at Ellensburg, Washington. <br />BOARD OF COUNTY COMMISSIONERS <br />K rr .lJNTY, WASHINGTON <br />Obie O'Brien, Cmnroissioner
