Officially enact Change Order #5 for the Teanaway
Paving Project.
In August of 20 15 The Director of Public Works
obtained verbal authorization of Change order #5 in
the amount of$133,228 .32.
The intent was to facilitate timely direction to the
contractor to proceed with the added and altered
work without a negative schedule impact.
Public Works Staff was directed to follow up with
the appropriate enabling documents, but through an
oversight, failed to do so.
This oversight was recently discovered while
processing final payment to the contractor for this
project.

TO APPROVE CHANGE ORDER #5 FOR
THE TEANA WAY PAVING PROJECT
WHEREAS: The Board of County Commissioners authorized road improvements to
Teanaway Road (Resolution 2015-104) in the amount of$3,000,300.00;
and
WHEREAS: Additional Costs are required for added and altered guard rail per the
revised contract plans; and
WHEREAS: The Contractor has offered unit price extensions for each; and
WHEREAS: It has been determined that the Change Order Total is $133,228.32. The
Director of Public Works recommends approval of Change Order #5.
NOW, THEREFORE BE IT RESOLVED that the Board of County Commissioners, in
the best interest of the public, does hereby approve Change Order #5 for
the Teanaway Paving Project for the amount of $133,228.32.

Justification:
Change Order #5
The County Engineer authorized design modifications to guardrail locations and requirements
following an on-site inspection of existing conditions in areas where the original topographic
survey data was insufficient to provide the detailed analysis of specific field conditions. This
eliminated the need for guardrail in some areas where it was deemed unnecessary and added or
modified guardrail design in other areas.
Cost analysis:
The contractor offered extensions of unit costs for added labor and materials, which was
deemed reasonable by Public Works.
