1 0. DESCRIPTION/JUSTIFICATION OF AMENDMENT:
T he Agreement Timeline is extended two months to allow for the pending approval of a controlled equipment
request waiver by DHS. Additions are denoted with grey highlight and deletions with strike through.
Key Personnel are also updated.
1 1. AMENDMENT TERMS AND CONDITIONS:
1. Change the Grant Agreement expiration date from June 30, 2018 to August 31, 2018.
2. The overall Grant Agreement amount of $50,548 remains unchanged.
3. Replace Amendment B Timeline Exhibit D with the Revised Timeline Exhibit D attached to this
a mendment.
4. Update Article 1, Department Key Personnel Exhibit A as follows:
a. Change Tirzah Kincheloe to Dalton Gamboa, dalton.gamboa(a~mil.wa.qov, 253-512-7044.
b. Change Michael Alston's title to Program Coordinator.
c. Remove Deborah Henderson.
d. Add Sierra Wardell, Section Supervisor, sierra.wardell(a~mil.wa.gov, 253-512-7121.
This Amendment is incorporated in and made a part of the Grant Agreement. Except as amended herein, all other terms
and conditions of the Grant Agreement remain in full force and effect. Any reference in the original Grant Agreement or an
Amendment to the "Grant Agreement' shall mean "Grant Agreement as amended". The Department and Subrecipient
acknowledge and accept the terms of this Amendment as identified above, effective on the final date of execution below.
By signing this Amendment, the signatories warrant they have the authority to execute this Amendment.
