ACTION REQUESTED:
The Board of County Commissioners' authorizes the signature of the Kittitas County Public
Health Department Administrator on the Health Care Authority Amendment 2. (Effective dates:
January 1, 2017 through December 31, 2018)
BACKGROUND:
The Health Care Authority Amendment 2 amends the contract manager information and replaces
the Statement of Work section in original contract.

Kittitas County Board of Health: N/A
Kittitas County Prosecutor's Office: Reviewed and approved on April 2, 2018.
Kittitas County Auditor's Office: Reviewed and approved on April 4, 2018.
RECOMMENDATION:
The Board of County Commissioners' authorizes the signature of the Kittitas County Public
Health Department Administrator on the Health Care Authority Amendment 2. (Effective dates:
January 1, 2017 through December 31, 2018)
