ACTION REQUESTED:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment 5. (Effective dates: January 1, 2015-
December 31, 2017)
BACKGROUND:
Consolidated Contract Amendment 6 Adds Statement of Work for the following program:
Emergency Preparedness and Response- Effective July 1, 2015
Office of Drinking Water Group A Program- Effective January 1, 2015
Exhibit B-6 Allocations, attached and incorporated by this reference, amends and replaces
Exhibit B-5 Allocations as follows:
Increase of $13,104.00 for a revised maximum consideration of $222,391.00
INTERACTION:
Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on March 25, 2016
Kittitas County Auditor’s Office: Reviewed and approved on April 5, 2016
RECOMMENDATION:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment 5. (Effective dates: January 1, 2015-
December 31, 2017)
HANDLING:
Clerk of the Board send notice of approval to the Auditor’s Office.
