ACTION REQUESTED:
The Board of County Commissioners approves the signature of the Public Health Administrator on the Child
Death Review Case Reporting System Data Use Agreement. (Effective Dates: April 4, 2016 through
December 31, 2020)
BACKGROUND:
The data use agreement is entered into by the Michigan Public Health Institute (MPHI) and Kittitas County
Public Health Department to establish terms and conditions for the collection, storage and use of data
obtained from the case reviews of child deaths submitted by Child Death Review teams in Kittitas county
and entrusted to the Michigan Public Health Institute as the Child Death Review Case Reporting System.
INTERACTION:
Prosecuting Attorney's Office: Reviewed and approved on March 9, 2016
Auditor's Office: Reviewed and approved on March 11, 2016
RECOMMENDATION:
The Board of County Commissioners approves the signature of the Public Health Administrator on the Child
Death Review Case Reporting System Data Use Agreement. (Effective Dates: April 4, 2016 through
December 31, 2020)
HANDLING:
Clerk of the board send notice of approval to the Kittitas County Auditors’ office.
