ACTION REQUESTED:
The Board of County Commissioners’ authorizes the signature of the Public Health Administrator on the
Consolidated Contract Amendment 5. (Effective dates: January 1, 2016- December 31, 2017)
BACKGROUND:
The Consolidated Contract Amendment 5 Adds Statement of Work for the following program:
Office of Immunization and Child Profile- Effective January 1, 2016
OICP- Prevention and Public Health Funds (PPHF) VTrckS-IIS—Effective January 1, 2016
Exhibit B-5 Allocations, attached and incorporated by this reference, amends and replaces Exhibit B-4
Allocations as follows:
Increase of $8,840.00 for a revised maximum consideration of $209,287.00
INTERACTION:
Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on January 13, 2016
Kittitas County Auditor’s Office: Reviewed and approved on January 18, 2016
RECOMMENDATION:
The Board of County Commissioners’ authorizes the signature of the Public Health Administrator on the
Consolidated Contract Amendment 5. (Effective dates: January 1, 2016- December 31, 2017)
